Meeting Minutes
June 1, 2009
Memorial Town Hall

MEMBERS PRESENT:         Mary Ricker, Mark Johnson

MEMBERS ABSENT: Martha Waite

OTHERS PRESENT: Andrea Crete, Board of Health Agent, Debra Sciascia, Recording Secretary, Randy Mizereck, Consultant

M. Johnson called the Board of Health Meeting to order at 5:00PM.

SEPTIC PLAN REVIEWS:

2311 Main Street: Heritage Design Group for Nancy Melendy. Septic upgrade for failing cesspool system. This will be a Presby system. There was concern with the proximity to the SAS. The applicant will be going before Con Com for approval on June 3, 2009.
Conditions:
1. Board of Health approval subject to Con Com approval.
2. The SAS must be staked by a registered engineer.
3. Instructions for maintenance must be given to the homeowner.

M. Johnson motioned to approve with conditions 1-3. M. Ricker seconded. Approved with conditions.

476 Wachusett Street: Quinn Engineering for Ted Beckner. This is for an individual component (septic tank replacement and relocation for a new house). The original system was replaced in 2002 for a mobile home, which will be removed from the property, and replaced with a new single family residence. The original plans were for a 4 bedroom system, and the new plans are for a 3 bedroom system. The applicant will be going before the Planning Board for approval to move the lot line between 476 and 482 Wachusett Street.

T. Beckner will need to fill out a new application and pay the $175 fee.
The Board made some preliminary recommendations. The existing septic system information needs to be added to the plans, going from a pump to gravity system.
R. Mizereck will call DEP to see if some of the 2002 components can be used with the new system.

M. Johnson motioned to table until R. Mizereck has time to properly review the plans. M. Ricker seconded Tabled.

482 Wachusett Street: Quinn Engineering for Ted Beckner. Septic System Upgrade. T. Beckner will be submitting a copy of the ConCom Order of Conditions. The applicant will be going before the Planning Board for approval to move the lot line between 482 and 476 Wachusett Street.

M. Johnson motioned to table until R. Mizereck has time to properly review the plans. M. Ricker seconded Tabled.

Wachusett Street - Map 31/Parcel 49 (Lot 9R): Winslow M. Spofford for Mark Peterson. New Construction for an approved expired design. The applicant paid the $175 permit fee.
Conditions:
1. Need to show additional deep holes on the plans.
2. Need to comply with new Title 5 requirements.
3. Need the dated soil evaluator's statement on the plans.

The Board will sign the new plans when submitted.

M. Johnson motioned to approve with conditions 1-3. M. Ricker seconded. Approved with conditions.

274 North Street: Request for New Construction perc test variance. There was a perc test done July 2008. The latest perc test was not complete. The Board recommended that the new soil and perc test be put on the plans, and the new plan must comply with Title 5 regulations.

M. Johnson motioned to approve. M. Ricker seconded. Approved.

596 Bullard Street: Lot 2: The applicant is requesting a one (1) year extension for new construction.

M. Johnson motioned to approve. M. Ricker seconded. Approved.
